JOB SUMMARY:
Responsible for the provision of safe, effective, and comprehensive medication management services. Deliver optimal medication therapy to patients with a broad range of disease states. Provide direct patient-centered care and integrated pharmacy operational services in medical offices practice via collaborative practice agreements. Participate in medication management while providing comprehensive and individualized pharmaceutical care to patients assigned. Provide pharmaceutical care services including assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent adverse medication events. Support in optimizing performance in value-based programs. Serve as departmental resource and liaison to other departments, clinical operations, decision support, and retail pharmacy operations. Provide medication and practice-related education/training to medical staff and residents. Responsible for dispensing prescriptions in retail pharmacy setting by following standard operating procedures for receiving, filling, dispensing, logging, and maintaining quality assurance of all prescriptions dispensed.
